From the North (Charlottesville): Take U.S. 29 South toward Lynchburg. Turn left on state Route 692/Plank Road. The clinic is located on the corner of U.S. 29 South and Plank Road in the Crossroads Corner Shops. Crossroads Family Practice is located on the second level around the back of the building, above Dr. Ho’s Humble Pie.
From the South (Lynchburg): Take U.S. 29 North toward Charlottesville. Turn right state Route 692/Plank Rd. The clinic is located on the corner of U.S. 29 South and Plank Road in the Crossroads Corner Shops, the same location as the Crossroads Country Store. Crossroads Family Practice is located on the second level around the back of the building, above Dr. Ho’s Humble Pie.
Call our office to make an appointment. To transfer your care, ask your current provider to fax your medical records to 434.966.3703.
Ask for prescription refills or renewals at the time of your visit. If you need renewals between visits, you can use MyChart, a secure online resource available for all UVA patients.
If you need to call us for a renewal or refill, we'll need the following information:
- Medical record number or date of birth
- Medication name and dose
- How often you take the medicine
- Supply needed (30 day, 90 day, etc.)
- Pharmacy name and phone number
Give us two business days to call in your prescription. Medications that need insurance pre-authorization may take longer.
Most results can be viewed using MyChart, a secure online portal that lets you view lab results and communicate with your healthcare provider. Your doctor may call you if there is an issue with your results.
Call 434.924.0000 and ask for the family medicine doctor on call. One of our doctors will contact you as soon as possible. If you have an emergency, call 911.
Give us 24-hours notice if you need to cancel or reschedule your appointment. Let us know as soon as possible if you expect to be late for an appointment.
