At Your Appointment
Your doctor will refer you for an appointment. You'll then meet with one of our attending doctors at your first visit. On follow-up visits, the entire heart failure center team will help manage your care.
Our Nurse Practitioner Clinic also ensures that you will be able to make an appointment to see a heart specialist even on short notice.
Appointments for the Hospital to Home Clinic are made for patients at the hospital prior to discharge.
At the Heart Failure and Transplant Center, our heart doctors and nurses work with you to treat and monitor your condition.
We take a comprehensive approach to healing your heart and can help you with everything from lifestyle changes such as diet and exercise to medications, device implants and even heart transplants.
We'll make sure your care plan is personalized for your needs and takes into account any other conditions you may have.

From North (Culpeper): Follow U.S. 29 South toward Charlottesville. U.S. 29 South turns into Emmet Street. Turn left onto U.S. 250 Business East/University Avenue. Take a sharp right at light onto Jefferson Park Avenue. Turn left on Lee Street. The Primary Care Center is located on your left next to University Hospital and across from the Emily Couric Clinical Cancer Center.
From South (Lynchburg): Follow U.S. 29 North toward Charlottesville. Take the exit onto U.S. 29 Business North/Fontaine Avenue. Continue straight onto Jefferson Park Avenue. Turn right to stay on Jefferson Park Avenue. Turn right onto Lee Street. The Primary Care Center is located on your left next to University Hospital and across from the Emily Couric Clinical Cancer Center.
From East (Richmond): Take I-64 West toward Charlottesville. Take exit 118B to merge onto U.S. 29 North toward Charlottesville/Culpeper. Take the U.S. 29 Business North/Fontaine Avenue ramp to Charlottesville. Turn right onto U.S. 29 Business North/Fontaine Avenue. Continue straight onto Jefferson Park Avenue. Turn right to stay on Jefferson Park Avenue. Turn right onto Lee Street. The Primary Care Center is located on the left next to University Hospital and across from the Emily Couric Clinical Cancer Center.
From West (Staunton): Take I-64 East toward Charlottesville. Take exit 118B to merge onto U.S. 29 North toward Charlottesville/Culpeper. Take the U.S. 29 Business North/Fontaine Avenue ramp to Charlottesville. Turn right onto U.S. 29 Business North/Fontaine Avenue. Continue straight onto Jefferson Park Avenue. Park in the Lee Street Parking Garage, across from Emergency Department. Your parking will be validated so there is no charge.
