North (from Waynesboro/Crozet): From U.S. 250/Rockfish Gap Turnpike, take state Route 151 South/Critzers Shop Road. Follow the road for about 11.5 miles. The clinic will be on your right next to the Wells Fargo bank.
South (from Lovingston): Take U.S. 29 North/Thomas Nelson Highway. Turn left onto state Route 6 West/River Road. Turn left onto state Route 151/South Rockfish Valley Highway. The clinic will be on your right next to the Wells Fargo bank.
East (from Charlottesville): Take I-64 West. Take exit 107 for U.S. 250 toward Crozet/Yancey Mills. Turn left onto U.S. 250 West/Rockfish Gap Turnpike. Turn left onto state Route 151 South/Critzers Shop Road. Follow the road for about 11.5 miles. The clinic will be on your right next to the Wells Fargo bank.
West (from Staunton): Take I-64 East toward Richmond. Take exit 99 for U.S. 250 East/U.S. 250 West. Merge onto U.S. 250 East. Turn right onto state Route 151 South/Critzers Shop Road. Follow the road for about 11.5 miles. The clinic will be on your right next to the Wells Fargo bank.
Call our office to make an appointment. To transfer your care, ask your current provider to fax your medical records to 434.361.2363.
Ask for prescription refills or renewals at the time of your visit. If you need renewals between visits, you can use MyChart, a secure online resource available for all UVA patients.
If you need to call us for a renewal or refill, we'll need the following information:
- Medical record number or date of birth
- Medication name and dose
- How often you take the medicine
- Supply needed (30 day, 90 day, etc.)
- Pharmacy name and phone number
Give us two business days to call in your prescription. Medications that need insurance pre-authorization may take longer.
Most results can be viewed using MyChart, a secure online portal that lets you view lab results and communicate with your healthcare provider. Your doctor may call you if there is an issue with your results.
Call 434.924.0000 and ask for the family medicine doctor on call. One of our doctors will contact you as soon as possible. If you have an emergency, call 911.
Give us 24-hour notice if you need to cancel or reschedule your appointment. Let us know as soon as possible if you expect to be late for an appointment.
