From North: Follow U.S. 29 South toward Charlottesville. U.S. 29 South turns into Emmet Street. Turn left onto U.S. 250 Business East/University Avenue. Take a sharp right at light onto Jefferson Park Avenue. The clinic is on the left.

From South: Follow U.S. 29 North toward Charlottesville. Turn right onto U.S. 29 Business North/Fontaine Avenue. Continue straight onto Jefferson Park Avenue. Turn right to stay on Jefferson Park Avenue. The clinic is on the right. If you get to the stoplight at Main Street, you have gone too far.

From East: Take I-64 West toward Charlottesville. Take exit 121 for state Route 20 toward Charlottesville/Scottsville. Turn right onto state Route 20 North. Turn left onto Elliott Avenue. Continue onto Cherry Avenue. Turn right onto Roosevelt Brown Boulevard. Turn left onto West Main Street. Take a slight left at the stoplight onto Jefferson Park Avenue. The clinic is on the left. If you go under the overpass, you have gone too far.

From West: Take I-64 East toward Richmond. Take exit 118B to merge onto U.S. 29 North toward Charlottesville/Culpeper. Take the U.S. 29 Business North ramp to Charlottesville. Turn right onto U.S. 29 Business North/Fontaine Avenue. Continue straight onto Jefferson Park Avenue. Turn right to stay on Jefferson Park Avenue. The clinic is on the right. If you get to the stoplight at Main Street, you have gone too far.

Parking

Park in the 11th Street Garage located next to the clinic. You should plan to arrive 30 minutes before your appointment to allow for construction and parking delays.

New Patients

Call us and choose Option 1 to make an appointment. Be sure to let the scheduler know that you're a new patient.

Do you have a MyChart account? You can ask for an appointment in the MyChart system.

Your Medications

Call us and choose Option 3 for medication refills. You'll need the following information:

• Medical record number or date of birth

• Medication name and dose

• How often you take the medicine

• Supply needed (30 day, 90 day, etc.)

• Pharmacy name and phone number

Give us two business days to call in your prescription.

Lab and Test Results

Lab results are available in MyChart, a secure online portal that lets you view lab results and communicate with your healthcare provider.

If you don't have a MyChart account, you'll get a copy of your results in the mail.

After-Hours Care

Call 434.924.0000 and ask for the on-call UMA doctor.

Cancellation Policy

Give us 24-hour notice if you need to cancel or reschedule an appointment. Patients that consistently cancel or miss scheduled appointments without notice may be dismissed from the practice.

Billing

Payments and copays are expected at the time of your visit.
