From North (Culpeper/Ruckersville): Follow U.S. 29 South. Merge onto U.S. 250 West/U.S. 29 South via the ramp to I-64/Lynchburg/Staunton/Richmond. Follow the road for about 3 miles. Take ramp to U.S. 29 North. Turn left onto Fontaine Avenue. Take the first right onto Ray C. Hunt Drive. Turn right to stay on Ray C. Hunt Dive. From South (Lynchburg): Follow U.S. 29 North toward Charlottesville. Turn right onto U.S. 29 Business North/Fontaine Avenue. Take the first right onto Ray C. Hunt Drive. Turn right to stay on Ray C. Hunt Drive. The practice is located in Fontaine Medical Office Building 1.

From East (Richmond): Take I-64 West toward Charlottesville. Take exit 118B to merge onto U.S. 29 North toward Charlottesville/Culpeper. Take the U.S. 29 Business North ramp to Charlottesville. Turn right onto U.S. 29 Business North/Fontaine Avenue. Take the first right onto Ray C. Hunt Drive. Turn right to stay on Ray C. Hunt Drive. From West (Waynesboro): Take I-64 East toward Richmond. Take exit 118B to merge onto U.S. 29 North toward Charlottesville/Culpeper. Take the U.S. 29 Business North ramp to Charlottesville. Turn right onto U.S. 29 Business North/Fontaine Avenue. Take the first right onto Ray C. Hunt Drive. Turn right to stay on Ray C. Hunt Drive. The practice is located in Fontaine Medical Office Building 1.

Parking

There's ample parking right in front of our building at 415 Ray C. Hunt Dr.

Please arrive 30 minutes before your appointment so you have time to register and check in before seeing your doctor.

New Patients

Call us and choose Option 3 to transfer your care to our clinic. If you're transferring from a facility outside of UVA, let us know so we can get your history and medical information from your previous healthcare provider.

Refills

Call us and choose Option 2 for the prescription refill line. You'll need the following information:

• Medical record number or date of birth

• Medication name and dose

• How often you take the medicine

• Supply needed (30 day, 90 day, etc.)

• Pharmacy name and phone number

Give us two business days to call in your prescription.

Lab and Test Results

Lab results are available in MyChart, a secure online portal that lets you view lab results and communicate with your healthcare provider.

If you don't have a MyChart account, you'll get a copy of your results in the mail.

Cancellation Policy

Give us 24-hour notice if you need to cancel or reschedule an appointment. Patients that consistently cancel or miss scheduled appointments without notice may be dismissed from the practice.

Payments and copays are expected at the time of your visit.
