Providing Excellent Healthcare at Home

For the third year in a row, Continuum Home Health Care, the Health System’s own home health agency, received high marks in the HomeCare Elite annual review, being named a top 500 agency.

This ranking is done by OCS HomeCare, which handles the mandatory benchmarking reporting to the Centers for Medicare and Medicaid Services for approximately 9,000 agencies nationwide.

Three areas of performance were included in the evaluation: quality of care, quality improvement and financial performance.

“Achieving this level of excellence truly takes a team effort that has involved the commitment of every single Continuum staff member, so it is wonderful to have this team's hard work recognized in this way,” says Continuum administrator Maggie Short.
