A primary care nurse practitioner, Caitlin Burchfield, RN, MSN, AG-PCNP, specializes in geriatrics and palliative care. In addition to her role as a clinical instructor in the UVA School of Nursing, Caitlin works with the Virginia at Home team, providing home-based primary care for homebound older adults.
Caitlin's clinical interests include:
- Climate change and health
- Examining improvement opportunities in transitions of care
- Geriatric frailty
- Home-based primary care
- Nursing & interdisciplinary health education
- Palliative care
After completing her nursing degree at UVA in 2010, Caitlin worked for three years as an inpatient cardiology nurse at UVA Medical Center. In 2013, she moved to the San Francisco Bay Area, working as a cardiology nurse at Stanford Hospital while pursuing nurse practitioner (NP) training at the University of California San Francisco.
During her last year of NP training, Caitlin was accepted to EdPACT (Education in Patient Aligned Care Teams), an interprofessional primary care residency program at the San Francisco Veteran’s Health Administration. She provided primary care for veterans while receiving advanced training in geriatrics, palliative care, and home-based primary care. After finishing the residency, she worked for the San Francisco Program for All Inclusive Care for the Elderly (PACE), providing intensive geriatric primary care to frail elders living in community.
Caitlin returned to UVA in 2019 as a cardiology nurse practitioner. In 2020, she paused her career to welcome her daughter, Eloise, into the world, and to begin pursuing her doctor of nursing degree.
