2019 Grant Recipients
We’ve approved the following grant proposals for funding in fiscal year 2019. Each proposed program addresses at least one of the stated priorities.
- Alzheimer's Association - providing the "Healthy Living for Your Brain" program
- Blue Ridge Area Food Bank - improving nutritious food access via the Mobile Food Pantry
- Boys & Girls Clubs of Central Virginia - addressing fitness and wellness among underserved girls ages 10-18
- Charlottesville Free Clinic - reducing health disparities and increasing primary care access
- City Schoolyard Garden - increasing food access and healthy living skills through Harvest of the Month and Friendship Court Farm programs
- Jefferson Area Board for Aging - empowering individuals through the Chronic Disease Self Management Program
- Jefferson Area CHiP - helping child-parent relationships with the Neighborhood Outreach Program
- Thomas Jefferson Area Coalition for the Homeless - connecting at-risk individuals to services
- Blue Ridge Medical Center - minimizing disparities in health outcomes through the Rural Health Outreach Program
- City of Promise - conducting a Community Health Survey of all residents
- Jefferson Madison Regional Library - Expand access to Wi-Fi hotspots and providing health literacy education
- Local Food Hub - supplying WIC beneficiaries with Fresh Farmacy services
- Mental Health & Wellness Coalition - training Certified Peer Recovery Specialists
- ReadyKids - broadening a healthy families mental health initiative
- The Women's Initiative - giving mental health counseling
- Piedmont Family YMCA - bolstering diabetes prevention
