- Welcome to the University of Virginia's Transitional Care Hospital, one of six long term acute care hospitals in Virginia. Our facility opened its doors in August 2010. We are a 40 bed hospital that specializes in caring for patients who have medically complex conditions. Being part of University of Virginia's health system allows our patients to receive full access to a wide range of clinical specialties, the latest technology, expertise, and services. At our transitional care hospital, you'll find caregivers and staff who are dedicated to providing the best environment possible for your treatment and overall well-being. I hope this tour will provide you with a facility overview and introduce you to the caring staff that will provide exceptional care. - Our lobby is the central hub for our services. Visitors would be greeted by a receptionists where they can obtain a visitor's pass. If the receptionist is off duty, please obtain a visitors pass from our second floor team station. In our lobby, you will find UVA imaging, our public restrooms, and outtakes cafe. Each floor features a visitation room where families can read, relax, or watch TV. Here in our facility, you and your loved ones are integral members of inter-disciplinary team. You will see and speak with your team on a daily basis. Our health care team includes physicians, nurses, patient care technicians, physical, occupational, speech, and respiratory therapist. - At Transitional Care Hospital, we have a therapy gym where we can help our patients get back to doing activities they love doing. Our equipment helps patients to stand up, to walk, any number of things they need to do to go back to the normal functioning they were doing before they got sick. - All of the nurses, PCAs, and other people here have been above my expectations. They care. They're very sincere and dedicated, and I have felt that I couldn't have found any place that they would have been as good for my wife. - The doctors were really nice, exceptionally friendly and personable. When they came into the room, it gave me the feeling that I was the only patient in the hospital. - All of our rooms are private. Each room is designed to promote healing. We have the latest technology, which can enhance your comfort. Our beds are all electronically operated and equipped with controls for lights, telephone, TV, and a call system to contact a member of your treatment team. - You never call that they don't show up to do whatever you need. - Many times there were a couple different nurses that almost seemed to have a sense of when I needed something, because when I was about ready to press the button to call the nurse, they would appear at the door. So I just found that really heartwarming. - In addition, some of our rooms are specifically designed to meet the special needs of our bariatric and dialysis patients. Let me introduce you to Amy Pennington who will talk more about our exceptional care and the patient experience here at the UVA Transitional Care Hospital. - Here at the UVA Transitional Care Hospital, we offer a warm, inviting, and welcoming environment for a patients, family members, and visitors. Our patient rooms feature an about me board, which list questions we ask patients to get to know them on a more personal basis. In the rooms, there are speakers to hear your favorite music, sun catchers to add to decor, and during special holidays, you may see a guest, for example, a leprechaun, a elf. Even Uncle Sam has been known to visit bringing a gift to our patients. Throughout the week during various times, you may hear soothing music from our volunteer hallway musicians, or you may get a visit from one of our many volunteers. - They play in the hall with a guitar and different things, and she hears it and it brings up her spirits. - Outside the patient rooms, you will find bird and squirrel feeders. There are also many blooming flowers and bushes planted along the hospital, as well as a patio for patients and loved ones to enjoy. - Outside of the hospital, it's a beautiful area. - And I just felt kind of at home. - If anybody have asked me, this would be my first recommendation, because they are wonderful. - The kind of care or attention that I received made me feel really good, because I felt like they cared about me.
