Can I Give a Family Member or Friend Access to My Patient Health Information in MyChart?

Patients can give a family member, friend, or caregiver access to their health information in MyChart. This is called proxy access.

Proxy access can be set up directly in MyChart through the Share My Record option or by completing a MyChart proxy form.

What Are the MyChart Proxy Options?

UVA Health offers the following proxy options:

  • Adult to Adult: For adults who want to give another adult access to their MyChart. Both adults must sign the form.
  • Adult Caregiver: For an adult who helps manage the care of a patient who cannot make their own healthcare decisions. Legal documentation or provider validation is required.
  • Parent/Legal Guardian to Minor Patient (age birth -12 years): For a parent or legal guardian requesting access to a child's MyChart. Legal documentation may be required in some situations.
  • Parent/Legal Guardian/Another Adult to Adolescent Patient (age 13-17): For a teen who wants to give a parent, legal guardian, or trusted adult access to their MyChart.

Can a Business and Related Purposes Get a MyChart Proxy to Review My Health Information?

MyChart is for personal and individual use. UVA Health won't approve MyChart access, including Proxy access, for any individual solely requesting such access for business and related purposes, even if they have legal guardianship (e.g., attorneys, social workers, department of social services, charities, advocates, case management, group homes, etc.).  MyChart is also not available to those that are incarcerated.
