How to Share & Access Your Records

We support new and ongoing patient care, case management, accounting, and other patient services. We also help support health services, research, legal affairs, and administrative affairs.

When You Need Your Medical Records

We want you to be part of the care team. That means having access to your medical information. You might need to get or share a copy of your medical records to:

  • See a specialist at a different hospital
  • Change doctors
  • Get a second opinion
  • Satisfy a legal request from a lawyer
  • Get insurance
  • Attend college

Share Your Records: DIY or By Request?

You can get printed or digital copies to share with another institution or yourself. But the easiest and fastest way is to use MyChart and let us do the sharing.

When we share medical records with another institution directly, we can:

  • Save you money, as it’s always free
  • Protect your privacy by making sure to give or get the correct records
  • Make sure they go to the right place

How to Request Medical Records

UVA Health offers multiple options for you to request medical records (including images). Medical records may be requested for the following reasons:

  • Patients requesting their own medical records to be sent to yourself or someone else
  • Parents or legal guardians of minor patients requesting medical records of the minor patient
  • Authorized patient representative acting on behalf of a patient
    • For deceased patients please include a copy of proof of executor or certification of qualifications to the request

You can request medical records one of these 3 ways:

  1. MyChart
    • Free, secure service that lets you view parts of your medical record, request copies online, message your provider, pay bills, and more
  2. Health Information Management Online Portal
    • Free, secure online tool to request your medical records. To verify your identity, you will be asked to upload a photo of your driver’s license or government-issued ID (via webcam or smartphone).
  3. Fax, mail, or email
    • Complete a Release of Medical Information Authorization Form (English or Spanish) and submit it by one of the following:
