  • Our service area covers a wide region of counties, including:

    • Albemarle 
    • Culpeper
    • Greene 
    • Madison 
    • Orange
    • Louisa 
    • Fluvanna 
    • Buckingham 
    • Nelson

    • Rockingham

    If you live outside of our 11-county service area, we'll work with you to identify and select a home health provider where you live.

    We also provide care in the cities of Charlottesville, Waynesboro, Staunton and Harrisonburg.

Home Health Services: Who Might Benefit

  • Continuum is Medicare/Medicaid Certified to provide services in the state of Virginia.

    Patients who have illness, acute illness, long-term health conditions, permanent disability or terminal illness may need home care. The most common conditions of patients using home care are:

    • Diseases of the circulatory system
    • Heart disease
    • Stroke
    • Diabetes
    • Hypertension or high blood pressure
    • Wounds including ulcers, bed sores or surgical sites
    • Post-hospital care

    Each patient receives care from a specific care manager who coordinates the patient's care with their physician and hospital discharge planners.

Home Infusion Product Delivery

The most common conditions of patients using home infusion services are:

  • Infections requiring IV antibiotics
  • Cancer treatment requiring IV chemotherapy
  • IV pain management
  • Nutritional needs requiring tube feedings or parenteral (TPN) support

 

Continuum can prepare and deliver home infusion products and supplies throughout Virginia and some neighboring states, including West Virginia, North Carolina and Maryland.
